If you're dealing with PCOS and facing challenges with fertility, you're likely familiar with how complex and frustrating the condition can be. While there is no one-size-fits-all solution, growing research suggests that certain dietary choices may influence hormone balance, insulin resistance, and reproductive health. One area receiving increased attention is dairy.

A 2025 scientific review analyzed the relationship between dairy intake, insulin resistance, and PCOS. The results offered meaningful insights for patients looking to manage their symptoms naturally and improve their chances of conceiving.

Impact of Dairy Products on PCOS

Dairy, particularly cow’s milk, can cause a strong insulin response, sometimes even more than carbohydrate-rich foods. Since insulin resistance is a key driver in PCOS, this effect may worsen hormonal imbalances, promote weight gain, and interfere with ovulation.

In addition, dairy contains naturally occurring hormones such as insulin-like growth factor 1 (IGF-1) and estrogens, especially in milk from pregnant cows. These hormones may elevate androgen levels and disrupt the delicate hormonal balance required for healthy ovulation.

One clinical trial included in the review placed patients with PCOS on a low-dairy, low-starch diet for eight weeks. The results were positive: participants experienced reductions in weight, insulin levels, testosterone, and waist circumference, along with improvements in hirsutism and blood sugar control.

Not All Dairy Is Equal

Some research suggests that low-fat and fermented dairy products like yogurt may have neutral or even positive effects on insulin sensitivity. However, other studies found that even low-fat milk could increase IGF-1 levels, particularly in patients already struggling with hormone dysregulation. The overall message? The impact of dairy is highly individual and may depend on factors such as type, quantity, and existing metabolic health.

Should Dairy be Eliminated from the Diet?

While the evidence is still evolving, this review supports the idea that reducing dairy, especially high-fat and high-insulin-stimulating forms, could be beneficial for some patients with PCOS. It may help address underlying insulin resistance and reduce symptoms that interfere with fertility.

That said, any dietary change should be approached with care and personalized to your specific health picture. What works for one individual may not be ideal for another.
